As a MyChart user, you will have access to many tools to take care of yourself and your loved ones. The following highlights some of the main benefits of being a MyChart user. Cleveland Clinic MyChart should never be used for urgent medical matters.

Test Results
- Click on the "Final Result" option to view the details of your recent tests and read comments if added by your physician.
Medications
- View a list of your current medications, request prescription renewals and learn more about your medications by clicking on "About This Medication".
Preventive Care
- View a list of procedures or immunizations recommended for you based on age, gender, and medical history. Due dates help you decide when to request tests or procedures.
Upcoming & Past Appointments
- View your upcoming appointments
- Click on a past appointment to view visit details of that appointment
Appointment Request Form
- Allows you to request a new appointment with one of your established physicians for your visit. Scheduling of new appointments will occur based on availability.
My Family's Records
- The MyChart • Caregiver service allows you to view family members' records. When clicking on a particular name, you will access that family member's MyChart account.
Questionnaires
- Some providers may send you questionnaires periodically. Questionnaires can be found in
either the "Message Center" or
"My Appointments".

Click on your MyChart Inbox, then click on the questionnaire and follow the instructions.
- "My Appointments"
Click on "Upcoming Appointments". Under the questionnaire sub-heading, click on the particular questionnaire link
and follow instructions.
