Yes___ No___
2. Is there a history of breast cancer in your family?

3. Do you have a history of endometrial or uterine cancer?

4. Do you currently have, or have you in the past had, venous thrombosis (blood clots in the veins of the legs or in the lung)? This includes thrombosis or blood clots during pregnancy or when taking birth control pills.

6. Do you have fibrocystic breast disease?
