Although anyone may request home care, all services must be ordered by a physician. Patients, physicians or family members can request services by calling 216.444.HOME (4663) or 800.263.0403, 24 hours a day, seven days a week.
If you are hospitalized, your physician in collaboration with the hospital discharge planner will provide you with the appropriate information for planning your home care services. Following your discharge from the hospital, a member of the Center for Connected Care team will contact you directly to arrange your first visit.
You do not have to be hospitalized to request our services. Patients at home or in an outpatient setting, emergency room, ambulatory clinic, assisted/independent living or skilled nursing facility may also be appropriate for Center for Connected Care services.
Our services are covered by Medicare, Medicaid and most major insurance plans. For you convenience, our staff will contact your insurance carrier to determine your exact coverage. You will be informed of the extent of this coverage and any potential financial liability before services begin.
Please note that Center for Connected Care does not provide emergency care. In a medical emergency, please call 911 or the local emergency number for your area.
