At Cleveland Clinic Canada, we are committed to providing evidence based solutions to a wide range of modifiable health problems, including obesity, diabetes and heart disease. Our multi-disciplinary team of physicians, case managers, exercise physiologists, registered dietitians and psychologists work together with you to help you take charge of your health, and your life.
In developing our programs, we recognized that simply providing a plan for a patient to follow on their own does not ensure success and neither would a strict set of predetermined rules that do not factor in their lifestyle. We understand the need for proactive medical care with targeted recommendations geared towards defined, measurable health outcomes. We also understand the need to empower you with the tools to take ownership of your unique lifestyle, not simply follow a program.
Cleveland Clinic Canada is unique because we are first and foremost a provider of clinical medicine. As such, it is in our DNA to treat complex and chronic medical conditions seriously and comprehensively and facilitate lifestyle change to improve specific health objectives. We treat lifestyle change according to evidence-based guidelines just as we would any other medical issue – collaboratively, objectively, patient-centred, and with a goal of consistent, sustainable outcomes.
We start by looking at the basics like medical risk factors, nutrition and physical activity habits. Then we review a patient’s lifestyle, environment and any sociological and physiological barriers that are preventing a successful lifestyle change. Once a patient is assessed, we use education, accountability and support to empower them to understand and confidently take charge of the things they need to do to make changes in their health.
